Geriatric Care Management in Rockland County, NY
Connections Care Management provides geriatric care management in Rockland County, New York, including Nyack, Suffern, New City and Pearl River. Co-founder Kim Hamilton is licensed in both New Jersey and New York, so we can help families on both sides of the state line. We visit in person, assess what your loved one needs, and help you carry out a clear plan.
Care management for Rockland County families
Rockland County sits just north of Bergen County, and many families have one parent in New York and adult children in New Jersey, or the other way around. Kim Hamilton, LCSW (NJ), LMSW (NY), C-ASWCM, brings more than 30 years of experience in hospitals and community agencies, with a focus on mental health and addiction. She is a natural fit for families who need help across the state line.
We start with a free first conversation. Then we do an in-person care assessment, usually three to five hours, and write a care plan. If you want, we stay involved with ongoing care management, regular visits and updates.
What families in Rockland County often face
- Hilly, car-dependent living. Much of Rockland is suburban and hilly, and many older adults depend on driving. When that stops, getting to appointments and stores gets hard. We help plan transportation and in-home help.
- Family on both sides of the border. New York and New Jersey have different programs, rules and Medicaid systems. We help you understand which state's options apply and how to coordinate care that crosses the line.
- Older homes and split levels. Many homes have stairs between every level. Aging in place often requires changes. See our aging in place home safety checklist.
- Mental health and addiction. Kim has deep experience helping adults and families with mental illness and substance use. Learn about our mental health care management and crisis intervention.
Hospitals and rehab in Rockland County
Rockland County's two main hospitals are Montefiore Nyack Hospital in Nyack and Good Samaritan Hospital in Suffern, which is part of Westchester Medical Center Health Network (WMCHealth). Helen Hayes Hospital in West Haverstraw is a New York State rehabilitation hospital that many families use after a stroke, injury or surgery. Some Rockland residents also use hospitals in northern Bergen County.
We can join hospital care meetings, help with the discharge plan, and go with your loved one to appointments as part of our hospital discharge planning and medical advocacy work.
Rockland County Office for the Aging
Rockland County's public aging office is the Rockland County Office for the Aging. It offers information and programs for older adults and caregivers, including meals, transportation and caregiver support. Contact details are on the county's website. Because New York programs differ from New Jersey's, we can help you understand what your loved one may qualify for and help with the applications through our benefits and paperwork service.
When a parent may need to move
Sometimes the safest plan is a move: to an apartment without stairs, to assisted living, to memory care, or to be closer to family in New Jersey. These are hard decisions, and they go better with a clear assessment and an honest look at options. We help families compare settings, visit with you and plan the move itself.
We never accept commissions or payment for referrals. Our only job is to find what fits your loved one. Our assisted living placement service explains how we approach it.
Rockland County towns we serve
We also visit clients in Spring Valley, Stony Point, Haverstraw, Orangeburg and other Rockland communities. If you live far away, our long-distance caregiving support keeps you informed. Contact Kim for a free first conversation.
