What a care manager does for Morris County families

An aging life care manager is a guide and advocate for an older adult and the people who love them. We look at health, safety, memory, mood, money and family dynamics, and then help you act. In Morris County that might mean helping a parent stay safely in a long-time home in Chatham, or helping a family choose an assisted living community after a hospital stay in Morristown.

We start with a free conversation, then do an in-person care assessment of three to five hours. You get a written care plan with specific next steps. Some families carry it out themselves. Others ask us for ongoing care management.

What Morris County families often face

  • Car-dependent suburbs. Outside the downtowns of Morristown and Madison, most of the county depends on driving. When a parent should stop driving, the whole routine has to change. We help plan transportation and in-home support.
  • Adult children far away. Many Morris County parents have children in other states. We provide regular visits and clear updates so you know what is happening. See our long-distance caregiving support.
  • Dementia at home. Families often try to manage memory loss at home for a long time. Our dementia care management helps you set up the right help and know when it is time for a change.
  • Choosing a community. The county has many assisted living, memory care and continuing care retirement communities. We give honest guidance through our assisted living placement service and never take referral fees.

Hospitals and rehab serving Morris County

Morristown Medical Center, part of Atlantic Health, is the county's largest hospital. Chilton Medical Center in Pompton Plains serves the northern part of the county, and Saint Clare's Denville Hospital serves Denville and nearby towns. Kessler Institute for Rehabilitation has a campus in Chester. Many families in Madison and Chatham also use Overlook Medical Center in nearby Summit.

We often meet families at the hospital, attend care conferences and help make sure a discharge plan is safe. Learn about our hospital discharge planning and medical advocacy.

Morris County's office for aging services

Morris County's public aging office is the Morris County Office on Aging, Disabilities and Community Programming, within the county's Department of Human Services. It is a good place to learn about county programs such as meals, transportation and caregiver support, and New Jersey's Aging and Disability Resource Connection. Contact details are on the county's website. We are glad to help you sort out which programs fit and complete the applications with you.

Planning ahead in Morris County

Many of the families we meet in Morris County are planners. They want to understand the options before a crisis forces a decision. That is a good instinct. A care assessment done early gives you a baseline, a list of risks to watch, and a realistic picture of what care may cost over time.

We can walk you through the difference between home care, assisted living and nursing home care, and how each might be paid for. Our guides on assisted living vs. home care and choosing a nursing home in NJ are good background reading before we talk.

Morris County towns we serve

We also serve Denville, Randolph, Mendham, Boonton and other Morris County towns. Read our aging in place home safety checklist, or contact us to talk with Lisa or Kim.
