Ongoing Care Management: Steady Support Over Time

Ongoing care management means we stay involved after the care plan is written: visiting regularly, watching for changes, coordinating doctors and providers, and keeping your family informed. It suits families who want a professional keeping an eye on things week to week, especially when needs are complex or family members are stretched thin.

What does ongoing care management look like?

Think of it as having an experienced professional on your loved one's side who already knows the history, the doctors, the medications and the people involved. When something comes up, you do not have to start from scratch explaining everything. We already know, and we can act quickly.

Ongoing support can include:

  • Regular visits. We see your loved one in person on a schedule that fits their needs, which may be weekly, every other week or monthly.
  • Monitoring for change. Weight loss, new confusion, low mood, unopened mail, a caregiver who seems overwhelmed. Small signs often come before big problems.
  • Medication and health services management. Making sure prescriptions are filled, taken correctly and reviewed by doctors, and that recommended tests and follow-ups actually happen.
  • Coordination with providers. Doctors, home care aides, therapists, pharmacies and care communities all talking to each other through one point of contact.
  • Family updates. Clear, honest reports after visits or whenever something changes, by phone, email or video.
  • Adjusting the plan. Updating the care plan as health, housing or family situations shift.

Who benefits most from ongoing support?

It is especially helpful when:

  • Your loved one lives alone, or with a spouse who is also frail.
  • There are several chronic conditions and several specialists.
  • Memory loss, mental illness or a disability makes it hard for them to speak up for themselves.
  • Family lives far away or has demanding jobs and children of their own.
  • In-home caregivers are involved and someone needs to oversee the quality of care.
  • There have been repeated hospital visits or falls.

How it works with us

Ongoing care management begins with a care assessment and a written care plan. Together we decide how often we should visit and what we will focus on. Some families want us involved in nearly everything. Others want us to handle specific pieces, such as overseeing home care or attending doctor visits, while they manage the rest.

The schedule is flexible. If your parent is stable, visits may be less frequent. After a hospitalization or during a decline, we can step up. Because we already know your loved one, we can respond quickly when there is a crisis, a hospital admission or a sudden change.

When needed, we also accompany your loved one to appointments through our medical advocacy service, and we oversee aides and agencies through in-home care coordination.

We also keep a close eye on money and mail. Older adults living alone are frequent targets of scams, pushy salespeople and people who show up offering help. Regular visits mean we notice unusual checks, new "friends" or unpaid bills early, and we can step in to protect your loved one from financial exploitation before real damage is done.

Why a social worker?

Lisa and Kim are both licensed clinical social workers with more than 30 years of experience each, certified in aging life care. Our training goes beyond tasks. We notice the emotional side of aging: grief, loneliness, family tension and the fear of losing independence. We have worked in hospitals, outpatient mental health clinics and community agencies, so we understand how these systems work and where people fall through the cracks.

What families tell us they value

Families often tell us the most valuable part is continuity. One person knows the full story and keeps track of it. Adult children say they appreciate being able to step back into the role of son or daughter, instead of acting as nurse, scheduler and case manager all at once. Parents frequently grow to look forward to our visits and come to trust us with concerns they might not share with their children.

FAQs

Ongoing Care Management: common questions

How often will you visit my parent?+

It depends on what your loved one needs. Some clients do well with a monthly visit plus phone check-ins. Others, especially after a hospital stay or during a period of decline, need us weekly or more. We set the schedule together after the assessment and change it whenever the situation calls for more or less involvement. You are never locked into a fixed number of visits.

Can you work alongside the family members who already help?+

Absolutely, and we prefer it. Families know their loved one in ways no professional can. We look for ways to support what you are already doing, fill gaps and lighten the load. Many families split responsibilities with us. For example, a daughter may handle grocery runs and visits while we manage medical coordination and oversee the home care agency.

Is ongoing care management covered by insurance?+

Medicare and standard health insurance do not pay for private care management. Many long term care insurance policies do, and we can help you understand what the policy requires. Some families pay privately because they find it costs less, overall, than repeated crises and hospital visits. We will talk openly with you about fees during the free first conversation.
