Elder Care Management in Passaic County, NJ
Connections Care Management helps older adults and their families in Passaic County, including Wayne, Clifton and Little Falls. We meet your loved one at home or in the hospital, figure out what is really needed, and help you carry out a plan. We are licensed clinical social workers with more than 30 years of experience each.
How we help families in Passaic County
Passaic County runs from dense cities in the south, like Paterson, Passaic and Clifton, to suburbs like Wayne and Little Falls and then to wooded lake communities further north. Families come to us for many reasons, but the common thread is the same: something has changed and you are not sure what to do next.
We start with a free call. Then we do an in-person care assessment that usually takes three to five hours. You receive a written care plan that spells out what to do and in what order. If you want us to stay involved, we provide ongoing care management with regular visits, monitoring and family updates.
Situations we see in Passaic County
- Two-family homes and multigenerational households. In Clifton and nearby cities, many older adults live with or near family, sometimes on another floor of the same house. That brings real support and real strain. We offer family counseling and help divide caregiving tasks fairly.
- Language and cultural needs. Passaic County is one of the most diverse counties in New Jersey. We take the time to understand your family's values and find providers who fit them.
- Suburban isolation. In Wayne and points north, a parent who stops driving can quickly become cut off. We look at transportation, day programs and in-home help.
- Applying for help. Many families here need guidance on state programs, including NJ Medicaid long term care. Our benefits and paperwork service and our guide to NJ Medicaid long term care basics can help.
- Mental health concerns. We work with older adults and younger adults living with mental illness, including help during a psychiatric hospitalization. See our mental health care management.

Passaic County's office for seniors
The county's public aging office is the Passaic County Department of Senior Services, Disability and Veterans' Affairs. It connects residents with county and state programs such as meals, transportation and caregiver support. Contact details are on the county's website. We are happy to help you figure out which programs your loved one may qualify for and handle the applications with you.
When to call a care manager
Families often wait until a crisis. It is easier when you call sooner. Good times to reach out include a new diagnosis of dementia or Parkinson's, a second fall, a parent who is suddenly confused after a hospital stay, unpaid bills piling up, or a stranger who seems too interested in your parent's money. Our work includes protection from financial exploitation, and we can step in quickly when something feels wrong.
If you are caring for a parent while working and raising kids, you do not have to figure it out alone. We can carry part of the load.
