Geriatric Care Management in Northern Middlesex County, NJ
Connections Care Management serves older adults, adults living with mental illness or disabilities, and their families in northern Middlesex County, including Edison, Woodbridge, Piscataway, Metuchen and Highland Park. We visit your loved one in person, assess their health, safety and support, and write a plan your family can act on. When you need someone on the ground over time, we stay involved.
Which Middlesex County towns do we serve?
We work in the northern part of the county: Carteret, Dunellen, Edison, Highland Park, Metuchen, Middlesex borough, Piscataway, South Plainfield and Woodbridge Township, including Iselin, Colonia, Avenel and Fords. If your parent lives in one of these towns, we can come to them at home, in the hospital, in rehab or in their care community. Not sure if your town is covered? Call us and ask.
How care management helps families here
Northern Middlesex County is busy and close-knit at the same time. Older adults live in post-war ranches and split-levels, two-family houses, garden apartments and senior buildings. Many adult children live nearby but are stretched between work, kids and a parent who needs more help each month. Others live far away and only see part of the picture. A care manager gives you one experienced professional who knows the local system and helps you decide what to do next.
We start with a free first conversation. Then we do an in-person care assessment, usually three to five hours, and write a care plan with specific recommendations. If you want continued help, we provide ongoing care management.
What families in this area often need
- Parents who moved here to be near family. Many older adults in Edison, Piscataway and nearby towns came from other countries or states to live near their children. They may not drive, may be more comfortable in a language other than English, and may feel isolated. We look for programs and providers that fit their culture and language.
- Rehab after a hospital stay. Short-term rehab is common after surgery or a fall. We help families choose a rehab setting and plan for the return home. See hospital discharge planning.
- Adults with mental illness or disabilities. We help adults living with mental illness or developmental disabilities, and their aging parents, plan for the future. Learn about our mental health care management and disability care management.
- Dementia care. We help families understand the stages, set up safe routines and choose memory care when the time comes. See our dementia care guide for NJ.
Hospitals where our clients are treated
JFK University Medical Center in Edison, part of Hackensack Meridian Health, is the main hospital for much of this area, and the JFK Johnson Rehabilitation Institute is on the same campus. Many families from Piscataway, Highland Park and Edison also use Robert Wood Johnson University Hospital and Saint Peter's University Hospital, the large teaching hospitals in New Brunswick. Some Woodbridge families, especially in Colonia and Avenel, use Robert Wood Johnson University Hospital Rahway.
We often join families for care meetings at the hospital or rehab, and we go with clients to specialist appointments through our medical advocacy service.
Middlesex County's Office of Aging and Disabled Services
The county's public aging office is the Middlesex County Office of Aging and Disabled Services, part of the Department of Community Services. It helps residents learn about programs for older adults and people with disabilities, and contact details are on the county's website. We can help you sort through what is available and complete applications through our benefits and paperwork service.
Supporting the whole family
Caring for a parent can bring out old tensions between siblings, or between a parent and the child who lives closest. We offer family counseling and mediation so everyone hears the same facts and shares the work. Our guide on holding a family meeting about elder care is a good place to start.
